8th Grade Waiver Form

U15 players who are still in the 8th grade will be allowed to play on a U14 team as a Guest
Player during the Fall Season only.

However, by USYS/OYSA Rules, these players will not be eligible to play with that team in
any Oregon Youth Soccer Association State Cup tournaments. These players must be
dropped from the team's roster before the team can participate in the State Cup tournament.
NOTE: guest players are not considered transfers when there is a limit of the number of
transfers you may have for a competition; but the standard fees must be paid. Also, no player
may play on more than one team in any one season.

The form below must be completed and returned to Oregon Youth Soccer Association,
attached to the registration/roster. Please make a copy for your records.
